WHEN: Wednesday, August 11th between the hours of 8:00 a.m. – 12:00 p.m.

General Information

- There is NO CHARGE for a sports physical at one of the above times. There will be a $25 fee charged (not billable to insurance) for sports physicals performed outside of the above times.

- There will be no makeup dates and late walk-ins will not be accepted.

- Students need to bring the KSHSAAA Pre-Participation Physical form with them, please complete the first page before coming to the physical. Forms can be obtained at www.kshsaa.org/FORMS/form-ppe.pdf.

- This will be a first come first serve basis, in which we will do our best to run efficiently and apologize in advance for any unanticipated wait.
