Health Information Needed for All Students

All students in all USD #337 buildings need to have specific information for the school nurse. Included in each student's folder should be the following: copy of birth certificate, Social Security number, immunization records and a health assessment.

If your child does not have one or more of these on file with Jennifer Collins, the school nurse, please get that information turned in to your child's school as soon as possible. Thanks!
